Greetings/comments from the hospital director

Heading toward 2025, integrated reform of medical care and long-term care are currently intensifying in Japan. A shift from hospital-based care to community-based care is being planned, and in keeping with this, I believe it is necessary for our hospital to attempt regional care. We have therefore launched a regional medical support project in 2017 as a bridge from the acute care hospital to the community, sending our medical professionals out for home-visit guidance before and after discharge connecting to community-based home-visit care, home-visit nursing, long-term care, etc.

I believe that seamless cooperation from acute care facilities to home-based care will be possible by confirming the condition after discharge in advance and providing the support according to the condition as well as taking over by ward nurses, certified nurses, etc. of their specialization to community-based care and its utilization in order to continue the care that was being received during hospitalization after discharge.

With the advancing age of the population and the increasing numbers with chronic diseases, hospitals alone cannot treat patients, and it is therefore necessary to build a medical system together with the community. I would like to do my best to support the community as much as I can with any requests the community might have.

In patient-centric medical care, I hope that each medical professional can provide self-directed care that is always thinking about the patient and makes the most of their special qualities. Including the work shift to other occupations as is spoken of in the discussion on reforming doctor’s working styles, we plan to go forward implementing and increase the efforts not only of doctors but also many other occupations to put patients first.

Medical technology is advancing every day and the environment surrounding medical care is changing, so our hospital must change also according to the times. But I believe that our mission to provide the medical care necessary for the region with fairness and impartiality and our desire to help patients will never change. With a strong focus on our mission and our feelings for the patients, we will strive to continue to be the hospital that the community needs.
