Coming to the hospital and procedures at the time of the hospitalization

Patients with hospitalization reservations should please go to the "General Reception" or "Admission/Discharge Support Center" on 1F.
When being admitted, please present the following at "General Reception" or "Admission/Discharge Support Center" on 1F.

Things to be presented by all patients at the time of the hospitalization

"Patient's registration card", "insurance card", "admission application", "surety document and medical expenses payment warranty", "To inpatients (record of hospitalization with the past 3 months)" "notification of requests regarding visitation"

Things to be presented only by the person in question

" Long-term care insurance card", " medical expenses public expenditure claimant certification", "hospitalization meal and medical expenses reduction and exemption certificate ", "certificate of qualification to receive medical expenses for infants, etc.", "high-cost medical care payment-in-kind system certificate"

Things to be presented only by those who wish to

"Hospital clothing application", "application for special room and consent form"

 

If there are changes to the "insurance card", "claimant certificate", address, contact information, etc. during hospitalization, please be sure to promptly notify "General Reception" on 1F.

Things to bring with you during hospitalization

  • hospital clothing (pajamas, etc.)
    If requested patients can be loaned hospital clothing from the hospital for a fee.
    At the time of admission, please notify the "General Reception" staff or during hospitalization please notify a nurse. They will explain the details.
  • Plain clothes
    Unless they obstruct treatment, examinations, etc., we recommend changing into plain clothes when getting out of bed. 惻Underwear, tissue paper, footwear, hangers, trash bins and other personal effects
  • Footwear
    Please bring footwear that covers the heel rather than slippers, etc. (exercise shoes, etc.)
  • Wash basin, towel, soap (body soap), shampoo, toothbrush, shaving razors and other toiletries, nail clippers, and denture cases when necessary
  • Small kettle, cups, chopsticks, spoons, and other things necessary for eating
  • Notebook, pen, and other writing utensils
  • Daily goods are sold at the shop on 1F.
  • Please write your name on things you bring with you.

Important points during the hospitalization

Please cooperate with the following matters for observation of your condition during hospitalization
Please refrain from excessive makeup.
Please remove nail polish from hands and feet, artificial nails, gel nails, etc. before hospitalization.
