Regarding medical certificates/certificates

If you wish to have a "medical certificate", "certificate", etc. issued, please go to Documents Reception at the "Various Consultations Plaza" on 1F.

Document Reception is open during the following hours. Please notify them as early as possible.

• Weekdays:8:30AM to 5:00PM

If you have a deadline for submission, etc., please consult with them in advance.
